Wellness in Action with Acorn to Oak Youth Services

The Mississauga Foundation was proud to award $22,975 through the 2024 Novo Nordisk Chronic Disease Grant to Acorn to Oak Youth Services. This funding supported their community-based diabetes prevention and management program for Black adults, addressing a critical health gap through culturally relevant education and support. The grant helped cover key expenses including workshops, transportation, printing, a coordinator’s salary, coaching, bi-weekly fruit and vegetable hampers, and health-related supplies.

The program had a strong influence on community members at risk of or living with Type 2 Diabetes. Participants gained knowledge on the importance of nutrition, food timing, meal planning, and physical activity. These learnings empowered them to make lifestyle changes that support long-term health. The regular group sessions also created a space for connection and support, positively affecting participants’ mental health. Many seniors, in particular, expressed appreciation for the practical tools and sense of community they received through the program.

This initiative was further strengthened by local volunteer support. A retired nurse contributed her time to monitor participants’ blood pressure and glucose levels, while others helped with logistics such as packing and distributing food hampers. The involvement of community members enhanced the program’s effectiveness and outreach.
